*New families will be contacted to schedule an interview and school tour. After your interview is complete, you will be notified of your application status within 24 hours.
The following list of vital documents will be required to finalize your enrollment determination. We will be happy to clarify any requirements for you.
All Students:
- Copy of Birth Certificate
- Copy of current immunizations
K-12: CA State Requirements:
7th Grade: CA State Requirements as of July 1, 2019
Additional Preschool CA State Forms:
- Identification and Emergency
- Pre-Admission Health Entry
- Physicians Report
- Notification of Parent Rights
- Notification of Personal Rights
Additional K-12 Forms:
- Copy of most recent report card (if applicable)
- Copy of most recent Standardized Test (if available)
- CA Report of Health Examination (required for Kindergarten)
