- 21/22 TK - 8th Grade Tuition & Fees
- 21/22 Distance Learning Tuition & Fees
STEP TWO: Click "ONLINE APPLICATION" below to apply
(Electronic payment of applicable fees is required to complete this step)
Note- We are currently unable to accept any more Preschool applications for the 21-22 school year. Any/all applications for preschool will only be considered for the 22-23 school year. (see that page here for pertinent information)
STEP THREE: Submit applicable forms to the office
- Enrollment Agreement Form (one per family)
- Emergency/Medical Form (annual per TK-12th student)
- Preschool Admissions Agreement (annual per child)
- Financial Agreement (annual per family - completed in office)
Vital Documents Required:
- Copy of Birth Certificate
- Copy of Current Immunizations (State Immunizations and Guidelines)
- Health Examination (TK or K Entrance Only)
NEW FAMILIES: Once an application has been received, we will contact you to schedule a tour with the principal. In Addition to the forms & vital documents, please provide the following if available.:
- Copy of current Report Card
- Copy of current Standardized Testing
- Copy of current IEP/504
PRESCHOOL FAMILIES: In addition to the enrollment forms and vital documents, the following forms are required by the state of California:
- Physician's Report (to be completed prior to start date)
- Identification and Emergency
- Pre-Admission Health Form
- Consent for Medical Treatment
- Notice of Parents Rights
- Notice of Personal Rights
- Caregiver Background Check Process
- Lead Poisoning Facts Flyer
